Overview

Under the general supervision of the collaborating physician(s) and under the administrative supervision of the VP of Physician & Diagnostic Services, is responsible for performing diversified clinical duties; and caring for and treatment of patients in the practices, Hospitalist program, Emergency Department, and Observation areas  in a courteous, respectful, and caring manner.

Qualifications

Education: Graduate of an accredited Nurse Practitioner program.

Experience: Three years or more as a Nurse Practitioner preferred.

Licensure: Licensed by the State of NH as a Nurse Practitioner. BLS (Basic Life Support) certification required, and must be maintained throughout employment. ACLS (Advanced Cardiovascular Life Support) certification required, and must be maintained throughout employment.

Responsibilities

Primary Care Duties

  • Conducts physical diagnostic, medical history taking, and therapeutic procedures within a primary care setting, in a professional manner in accordance with established standards of care and practice.
  • Formulates an appropriate plan of care based on physical exam, patient history and clinical findings, and collaborates with physician(s) as necessary.
  • Contacts patients with follow up information on testing, in accordance with practice and Hospital policy and procedure.
  • Recognizes and reports relevant information that requires urgent attention, and ensures follow through.
  • Writes orders including diagnostic tests, referrals, and medication.
  • Answers patient questions and clarifies concerns regarding medications, diet, activity, and health status.
  • Provides and promotes effective and compassionate care for patients and families.
  • Communicates with referring specialists.
  • Works collaboratively and cooperatively with all members of the multi-disciplinary teams.
  • Make medical diagnoses and institute therapy or referrals of patients to the appropriate healthcare facilities, agencies, other resources of the community, or other physicians.
  • Coordinates treatment with patient and family members.

Hospitalist Program Duties

  • Provides full spectrum of inpatient care including, but not limited to, admitting and discharging patients, rounding on patients, and medicine consultations.
  • Provides medical evaluation and treatment of hospital patients.
  • Works collaboratively and cooperatively with all members of the multi-disciplinary teams.
  • Works closely with Utilization Management and Nursing Managers to reduce length of stay and improve quality of care by utilizing resources efficiently and effectively.
  • Collaborates with nursing, support personnel, Physicians, and administration in matters affecting patient care following policies, procedures, standards of care, standards of practice, standards of infection control, etc.
  • Coordinates treatment with patient and family members.
  • Emergency Department & Observation Area
  • Conducts physical diagnostic, medical history taking, and therapeutic procedures within the emergency department setting for appropriately triaged patients, in a professional manner in accordance with established standards of care and practice.
  • Formulates an appropriate plan of care based on physical exam, patient history and clinical findings, and collaborates with physician(s) as necessary in the Emergency Department and Observation setting
  • Conducts physical diagnostic, medical history taking, and therapeutic procedures within an Observation setting, in a professional manner in accordance with established standards of care and practice.
  • Contacts patients with follow up information on testing when necessary and in accordance with established Hospital policy and procedure.
  • Recognizes and reports relevant information that requires urgent attention and ensures follow through
  • Writes orders including diagnostic test, referrals, and medications.
  • Answers patients’ questions and clarifies concerns.
  • Provides and promotes effective and compassionate care for patients and families.

Organizational Responsibilities

  • Utilizes the current Electronic Medical Record or appropriate documentation methodologies to complete the documented Best Practice Workflows accurately.
  • Documents physical findings and patient response on appropriate worksheets and medical record.
  • Participates in staff meetings and cooperates with physicians and coworkers to achieve multiple departmental objectives. Reports concerns and problems as they arise.
  • Ensures confidentiality of employee, legal, client/patient, budget and all organizational matters.
  • Keeps knowledge base current by attending educational programs.
