  • Overview

    Under the general direction of the Cardiac Rehab Program Coordinator, is responsible for supervising Cardiac Rehab exercise sessions and directing and prioritizing the work of the program staff, which may include MA/LNA/Exercise Assistants.

     

    This is a per diem position

    Qualifications

    Education: Graduate from an accredited School of Nursing, BSN preferred.

     

    Experience: 3-5 years of experience in cardiac care, cardiac rehab or intensive care nursing. Must be proficient with cardiac monitoring systems (Scott Care), electronic documentation and working collaboratively with multidisciplinary teams. 1-2 years of Supervisory experience preferred. 

     

    Licensure: Current New Hampshire Registered Nurse License.  Certification in Cardiac Rehab Nursing or Critical Care (CCRN) preferred.  Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) required.

    Responsibilities

    Cardiac Rehabilitation Program Supervision and Patient Education

    • Effectively communicates, collaborates and appropriately interacts with the Cardiac Rehab Program Coordinator and Cardiac Rehab Program staff; the Cardiac Rehab Medical Director, Director of Rehabilitation & Diagnostic Services, Dietician, and all interdepartmental staff.
    • Reviews and complies with all departmental policies and procedures. Complies with CMS regulations, and state and national Standards of Care for Cardiac Rehabilitation. Reviews professional AACVPR guidelines and recommended best practices to stay current with advances in Cardiac Rehab and in preparation for program certification.
    • Interviews patient candidates for admission to Cardiac Rehab program. Explains thoroughly to patients and family, all aspects of the program, including both the exercise component and the educational component.
    • Supervises each exercise session, utilizing a thorough knowledge of cardiac monitoring. Responds to changes in patient status in accordance with ACLS guidelines and Department Policies and Procedures.  Adheres to exercise prescription, Individualized Cardiac Rehab Treatment Plan and Huggins Hospital patient safety protocols.  Provides appropriate patient monitoring and supervision to detect deterioration in clinical status and provides timely feedback to the Medical Director and/or Supervising Physician, as well as patient’s referring physician, to enhance effective medical management.

    Patient Advocacy

    • Develops and/or implements and maintains an individualized, multidisciplinary Individualized Cardiac Rehab Treatment Plan (“ICTP”), including awareness of and sensitivity to individual cultural needs.
    • Promotes patient's independence by establishing patient care goals; educating patient, friends, and family to understand patient’s condition, medications, and self-care skills; and answering questions. Leverages multidisciplinary team to meet patient’s health and rehab goals.
    • Utilizes appropriate resources for non-English speaking and limited English proficiency (LEP) patients, and for hearing and sight impaired individuals, and/or healthcare illiteracy.
    • Takes proactive steps to recognize and report suspected abuse including child, geriatric, sexual, and/or spousal abuse, and reports same to facility supervisor and/or appropriate authorities.

    Quality

    • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; and by making or recommending necessary adjustments.
    • Follows Huggins Hospital’s and Cardiac Rehabilitation’s mission, philosophies and standards of care.

    Professionalism

    • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications.
    • Establishes professional network and/or participates in professional society for specialty.
