  • Overview

    Under the immediate supervision of the Sr. Director of Community & Population Health, is responsible for organizing, supporting and coordinating all activities under Huggins Hospital’s federal Rural Health Network grant. In addition, this position will be responsible for coordination and operations of Huggins Health Neighborhood to provide community-based screening, referral and navigation of residents to address health-related social needs.


    Education: Bachelor’s degree in health, public health, social work, a related field or a creative combination of education, experience, background and interest.

    Experience: Candidate should have strong verbal and written communication and interpersonal skills and experience building positive and productive relationships with partners and stakeholders. Experience coordinating projects or programs; planning and providing administrative support to meetings and conferences; and managing communications for workgroups or committees.


    Coordination of Rural Health Network advisory board and Network activities.

    • Plan, organize, coordinate, support and follow up on all Network Board meetings including production of agenda and minutes.
    • Plan, organize, coordinate, support and follow up on community stakeholder meetings and conferences.
    • Support and maintain federal grant tracking and reporting requirements including grant project budget, time tracking, costs and effort toward tasks.
    • Support and participate in Rural Health Network quality improvement activities.
    • Maintain Rural Health Network project plan to track progress and completion of all related activities and deliverables.

    Huggins Health Neighborhood operations

    • Under the direct supervision of the Sr. Director, develop tools and processes to launch Huggins Health Neighborhood social determinants screening, referral and navigation operations.
    • Coordinate operational and administrative activities of Huggins Health Neighborhood and Community and Population Health Department.
    • Perform screening, referral and navigation of residents to services to address health-related social needs, bridging the gap between the community and the health and social service systems..
    • Works closely with Huggins Hospital’s primary care embedded care coordination team to identify client and patient needs and provide comprehensive wrap-around services for clients and patients.
    • Collect data to track services provided and produce related reports.
    • Participate in data-driven quality improvement activities and maintain quality assurance of all deliverables

    Contribute to development of Rural Health Network and Huggins Health Neighborhood deliverables and work products

    • Contribute to Rural Health Network grant deliverables and work products including required project reports, governance processes, gap analyses, strategic planning, community listening sessions, and other activities.
    • Collect and analyze data and perform descriptive statistics as required to present qualitative and quantitative findings via written materials such as reports and presentations.
    • Gather, analyze and summarize relevant community and public health data and assist development of grant proposals to support program operations and expansion.

    Communications and relationship management

    • Utilize community organizing skills to build relationships with Huggins staff and community-based organizations to engage partners and the community.
    • Responsible for Rural Health Network communications to keep Network Members informed and solicit their input.
    • Maintain frequent email communications to keep the Advisory Board informed and on track with Rural Health Network activities.
    • Work with the Community Relations Department to produce public communications about Huggins Health Neighborhood including written materials, web stories, social media and other relevant vehicles to inform and engage the community
